By completing this form, vendors participating at the Cocktails In Havana event on August 21, 2015, acknowledge that they will abide by the following guidelines.

2. All our booth attendants will 21 years of age or older.

3. I/We will have someone attending our booth at all times during the event, starting at 5:00pm.

peterk@contactescrowinc.com by June 23, 2015.

5. Your tax-deductible donation of product and/or services will be promoted throughout the evening and enjoyed by more than 500 successful, and influential guests.

6. The event team will provide all serving needs for guests such as tables, plates, utensils, napkins.

7. You may bring your banners, menus, business cards and other items to promote your business.
